Short answers, each with its source.
The AAO's care guide describes brushing it every day with a toothbrush kept for it and dish soap, using lukewarm water, and soaking it in a cleaning tablet or solution once a week for 10 to 20 minutes.
The AAO's retainer-care guide does not cover vinegar, so ask your orthodontist before you try it. The same guide does describe a soak in hydrogen peroxide mixed with water for stains.
The makers say high-frequency sound in water makes tiny bubbles that loosen buildup, and owners commonly report a cleaner appliance than with brushing alone. The AAO lists ultrasonic retainer cleaners and solutions among the ways to clean a retainer.
Jewelry cleaners such as the Magnasonic MGUC500 are not marketed by their makers for dental appliances. Check with your orthodontist and the appliance maker's care guide before you use one.
Zima says its tablets are optional and recommends one if you wear your appliance for more than 6 hours at a time. Zima also says some other brands' tablets damaged and corroded its tank.
The AAO says an old or dirty case can harbor bacteria, and that it is time for a new one when you see cracks or wear, or when the case still smells after cleaning.
